New Patient Registration

We register new patients who live in Selby or in a village within a 7-mile radius of the practice. To help us give you the best service possible we would encourage you to do this as soon as possible.

When you register with the practice you will be asked to complete a registration form known as a GMS1 and a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

You complete these forms at reception, or alternatively click on the link below. Please return the completed forms to reception, or email to them to

It would help us when processing your application if you included some photo ID and proof of residency, however this is not a legal requirement, and your registration will not be delayed if these aren't provided.
